One of the arguments justifying the use of anti-antigenic agents in combination with radiation therapy is a direct link between endothelial cell apoptosis and tumor response. Apoptosis of endothelial cells is mediated by rapid  generation of the sphingolipid ceramide through the hydrolysis of cell membrane sphingomyelin by the AT 406 structure sphingomyelinase (ASM) enzyme.45 Garcia-Barros et al. have published the results of preclinical experiments in which MCA/129 fibrosarcoma and B16F1 melanoma tumors were grown in both wild-type and ASM knockout mice.46 Radiation therapy was found to induce regression of both tumor types in wild-type, but not ASM knockout mice. Endothelial cell apoptosis occurred in the tumor vasculature of WT mice, but was inhibited in ASM knockout mice, demonstrating that endothelial cell death directly affects tumor radiosensitivity. However, the above experiments were conducted using single high-dose radiation (>15 Gy), which would be relevant only to hypofractionated stereotactic radiotherapy. Using conventional fractionation (2 Gy per fraction), the importance of endothelial cell killing for tumor radiocurability has been disputed and must be carefully investigated. Despite the possibility that significant endothelial cell apoptosis likely occurs in a dose-dependent fashion, starting above a certain threshold, any result discards the possibility that the accumulation of 2 Gy fractions will not provoke endothelial cell death.
